2. Forms
Important forms required for new clients will be assigned to you via your client portal and can be signed electronically. However, if you want a preview or a copy of my Informed Consent and Fee Schedule click the PDF icon below. My Privacy Practices, as well Square's Privacy Policy, can be found under the Services page (see tabs above). New clients should review these and make note of any questions or concerns you may have, I will be happy to discuss them with you.
3. Online Functions
Once you have attended your initial appointment, you will have access to schedule and cancel appointments online through your online client portal. I do not accept online cancellations with less than 24 hours notice (see Fee Schedule or Rates page), so please call me if you have to cancel with short notice. Clients cannot schedule online less than 15 hours ahead of time, so if an appointment is needed for next/same day, please call me. I request that clients set up no more than two (2) future appointments at a time.
